At the end of the nomination form you will be asked two questions:
- How has the nominee/s demonstrated the value in their behaviours and actions and how have they performed above and beyond? Please include what they did and why.
- What have been the outcomes, or the impact of their actions and behaviours on patients, peers or the organisation? Please use measures where appropriate i.e. overall rating of treatment and care, patient survey score, time saving, reduced health & safety risks, team productivity, efficiency, morale etc.
Consider answering these in a word document so you can paste your response into the nomination form.
If you need to include more than one attachment as evidence, please email them through to awards.awards@healthscope.com.au and include the nominees details.
